The next stage is to try other bathrooms that make you feel sort of protected and step up the frequency to these until they become "safe".

Keep in mind that this is a gradual process so you shouldn't overwhelm yourself or try and take on a lot of situations at once. You want this to be a positive experience and not something that makes you feel so anxious that you quit. You definitely get to feel a bit anxious but that's acceptable and totally normal.

Self esteem problems do come into play here as most people suffering may have guessed. The process of building up your self esteem will establish big improvements in your therapy to overcome shy bladder. Anxiety and bashful bladder go together like ebony and ivory. The sufferer is visibly anxious about peeing in a community restroom and their fear they feel builds to a point where they can not go pee. What happens is the bladder will become a bunged fist for which no urine will flow through and your job is to make an effort to defeat this.

A neat secret for being able to use the bathroom in a public place is to carry a Walkman or MP3 player with you. Merely put on your earphones and blast the volume before you enter the bathroom. If you make it loud enough it will absolutely cover up the sounds from other folks you could be hearing in the bathroom. So crank up the sound, shut your eyes and let your thoughts think you are in the relaxation of your own residence using the toilet.
